Hey guys! Another crap install we walked into! There inspection form was also filled out incorrectly. I had my brother rewire this one (he is in training to become Nicet certified) Not as clean as I would want but hes heading in the right direction. Also the prev company had lost the connector for the remote sink and jammed another connector in. I have ordered another one from firelite. Horn strobe was set to high and installed in a bathroom! Panel not grounded per mfg! Below the pics is a list of violations I gave to the customer.EXPLANATION OF VIOLATIONS LOCATED AT THE ABOVE NAMED PROPERTY At the above named property the following list of deficiencies and violations have been identified And are listed below. Some of these codes have been put into effect by ADA (American Disability ACT) which are federal guidelines. Codes, violations or guidelines not followed carry extremely heavy fines however this is more apparent when an emergency has happened in the building and someone was injured or at worst, killed. All code references are indemnified Using the NFPA 72 2002 Edition (National Fire Alarm Code) which is currently use in CT. General electricians and low voltage installers not familiar or properly trained generally do not have fire alarm expertise while they try and do the best job they can most are unaware of the stringent codes well beyond the NFPA 70 (National Electrical Code) and some automatically conduct themselves unethically. Fire alarm specialists that are Nicet certified in fire alarm engineering are extremely aware of these codes due to the fact rigorous testing must take place and we must have at least 5 years of field experience, commissioned systems and also secured fire marshal references. We are constantly audited by NICET to ensure we are upholding the primary goals to protect life and property. Violations committed by the previous alarm and/or electrical contractor have been highlighted in yellow. Violation: 4.3.1: Equipment shall be listed for the purpose used. Fire alarm components shall be installed with manufactures’ published instructions. Explanation: 1) The mounting of the horn/strobes, pull stations, smoke detectors and heat detectors are in direct violation With 4.3.1 because the manufacture is specifying that all devices be mounted to a back box. Attached to this document you will find a PDF highlighted displaying the mounting instructions of the devices. 2) The manufacture of the fire alarm manual specifically indicates within the instruction manual to mount the operating instructions that shall be framed adjacent to the control panel Repair: Peace Of Mind Security will remount all devices and devices on a drop ceiling will be properly supported by a UL listed box for ceiling grid use and a 20lb support wire attached to the building structure. For the instructions Peace Of Mind Security will frame and display the operating instructions of the control adjacent to the control panel. Violation: 760.8 Mechanical Execution of Work. Fire alarm circuits shall be installed in a neat workmanlike manner. Explanation: The installation of the conductors into the control panel are unethical, not labeled, and some conductors not properly stripped. Repair: Peace Of Mind Security will rewire the control panel and label each circuit in a very neat workman like manner. Peace Of Mind Security LLC 40 Navarro Rd. East Haven, CT 06516 ELC: 0188305-E1 ELC: 0198097-L5 203-901-4350 Joseph Montuori CET LVL III Nicet Certified Engineering Technician ID: 119800 Property Location: 230 Roosevelt Dr. Monroe, CT Page 2 Violation: 4.4.1.4.2.3 Location of disconnecting means shall be identified at the control unit and marked in red “Fire Alarm Circuit” Explanation: This code requirement requires the installed to identify on the panel which circuit breaker is used to disconnect power To the panel for service and this circuit must be labeled at the electrical panel in red fire alarm circuit and shall also be mechanically protected by a breaker lock. Repair: Peace Of Mind Security will properly identify and label disconnecting means. Violation: 760.9 Fire Alarm Circuit and Equipment Grounding. Fire alarm circuits and equipment shall be grounded in accordance with Article 250. Explanation: The fire system is not properly grounded per the manufactures instructions. Repair: Peace Of Mind Security will properly ground the fire panel Violation: 4.4.4.1(3) Equipment shall be able to operate at temperatures of 32F-120F Explanation: Currently the panel is showing a supervisory because the hardware store is not heated causing the Smoke detector to operate below its required range of 32F. Repair: Either heat the vacant space or Peace of Mind Security will submit a document of relief of the code to the fire marshal to temporary replace the smoke detectors with heat detectors until occupied. Violation: Transmission Channels: Any system employing DACT (Digital Alarm Communicator Transmitter) shall employ one telephone line (number) In addition one of the following transmission means shall be employed. (1) A second telephone line (number) (2) A cellular telephone connection. 8.5.3.2.1.4 Explanation: System is being backfed from one telephone line. Repair: Customer will have a 2nd line brought to the fire panel by the telephone company. Violations: 4.5.2.1 A record of completion shall be prepared for each system by a qualified person 4.5.3.1 A complete record of tests and operations shall be kept until the next test and 1 year after 4.5.3.3 Supervising stations shall maintain records of all signals and tests for not less than 1 year 10.2.2.1The owner is responsible for system testing, inspection & maintenance 10.2.2.2 Delegation of this responsibility shall be in writing 10.4.1.1 Acceptance testing shall be done on all new systems 10.4.3.2.1 Smoke detector sensitivity shall be checked within 1 year of installation 10.4.3.2.2 Smoke detector sensitivity shall be checked every alternate year thereafter 10.4.3.2.3 If the smoke detector remains within the listed sensitivity range, then the sensitivity tests can be extended to 5 years 10.4.2.6 Sensitivity may not be tested using a device that administers unmeasured smoke concentration Explanation: There is no evidence of inspections, testing, sensitivity testing, or delegation of responsibility. Page 3 Repair: Peace Of Mind Security will pull a permit to make repair and provide a record of completion and perform semi annual inspections per the NFPA 72. Violations: 7.3.1 All notification appliances shall be listed for the purpose they are used 7.4.1.2 The total sound pressure shall not exceed 110 dBa anywhere in the occupied area Explanation: The device in the Dunkin Donuts bathroom is a horn/strobe and because it is in a small area the decibel Rating exceeds 110 dBa per the ADA and is above the pain threshold that can cause ruptured ear drums. Only strobes are installed in bathrooms for visible notification only. Repair: Peace Of Mind Security will replace all horn/strobes in bathrooms with a standard strobe.
