Important paper just came across my desk. This was a collaborative study with surgeons and endocrinologists participating. The goal was to compare intensive medical therapy vs surgery for diabetes in people that are slightly obese but generally lower than what we would typically approve for surgery. This was a one year study. Results are dramatic. Despit diet, behavioral therap, and all the latest meds the medical group only achieved an A1C less than 6 (meaning excellent diabetic control) in 9% vs 44% in surgery group. Surgery group had lower triglycerides, lower LDL, lower blood pressure and lower weight. They make a big deal in the article about complications for bypass which were very few, but they fail to note the long term complications that the medical group are sure to acquire if they continue to have elevated sugars. All in all this is a very strong paper in support of gastric bypass for patients with diabetes but BMI less than 40. ncbi.nlm.nih.gov/pubmed/23736733
