A network of community health centers is seeking a passionate and dedicated Family Nurse Practitioner to join their new York, Pennsylvania health center, where a low income community is served. Patients are in need of your competent and sensitive care. Their interdisciplinary model is dynamic and includes Primary Care, Behavioral Health, Family Planning, and Social Work in order to provide the best wrap around care for vulnerable people. The Family Nurse Practitioner will provide all aspects of primary care including family planning services. Responsibilities include but are not limited to: medical assessment, diagnosis, treatment, referral and follow-up. Applicants must be a licensed practitioner, have at least (1) year of experience in a primary care setting preferred and a Master’s degree in Nursing.
