Admissions Liaison Nurse Admissions Liaison Nurse Penylan House Nursing Home, Cardiff Monday to Friday – 38.5 hours per week £26,910 - £28,910 per annum (depending on experience) Linc Care operates across South Wales and provides homes and services to older and vulnerable people in the not-for-profit sector. Penylan House has a history of delivering excellent care to the local community since 1955. We are delighted to announce the completion of Phase 2 of Penylan House and the addition of a further 43 bedrooms. Penylan House has been providing residential, nursing, dementia and convalescence care for more than 50 years. The fully redeveloped modern, purpose-built Nursing Home now has 75 single, ensuite bedrooms and there couldn’t be a more exciting time to join us. We have adopted the ‘Eden Alternative’ approach to care, and place the individual at the heart of our care philosophy. We are looking for an experienced Qualified Nurse to lead our admissions process across our two Nursing Homes in both Cardiff and Newport, and deliver outstanding customer service to our residents and families at what can be a very challenging time. The Admissions Nurse is a newly created position which will take responsibility for the day-to-day management of a comprehensive high quality and effective bed management service which works in close liaison with the dis-charge, placement, and social services teams of our customers. The role will also act as the first point of contact for elders and their families, and will support them through the admissions process by building trust and communicating in a clear and confident manner. Working in conjunction with the Registered Manager and/or Clinical Lead, the Admissions Nurse will use a variety of clinical nursing tools to assess the individual health care needs of each person and advise on the most appropriate level of care. An in-depth knowledge of the referral process within a nursing home setting is required, alongside the ability to liaise on a daily basis with relevant stakeholders including the NHS, local authorities, and GPs. You will be required to travel between our homes and you will therefore need a full driving licence, with access to a vehicle insured for business use. The working hours for the role are envisaged to be predominately Monday-Friday during normal office hours, however applicants are advised that there may be times where the normal duties of the role mean that evening or weekend work may be required, or where the Admissions Nurse will be expected to support the operation of the home by providing cover on the floor.
