Blocking out dark discolored teeth is tricky...especially when trying to be minimally invasive and conserve healthy enamel. This patient presented with a history of 3 separate devital bleaching procedures, so we were concerned about additional bleaching attempts. I need to create enough space to block out the discoloration with opaquers and still leave room for the Microfill surface. My depth gauge is 0.3 mm. Only at the cervical millimeter or two, am I into dentin, so my prep is almost entirely in enamel. The KEY to subsurface opaquing is to place a thin layer of Clear Liquid Resin (#Cosmedent) over the tooth structure after the tooth has been etched and the adhesive has been placed and polymerized. The Clear Liquid Resin is light cured for 20 seconds. This layer will reduce the surface tension so it will be easier to place your opaquer directly to the tooth structure without pooling of the opaquer. I will post more pics with this case later...
