Douglas School District will hold Kindergarten Registration and Screening for the 2014-2015 school year during the week of March 3-7. This is by appointment only and children must be present. To be eligible for kindergarten, children must be age five years old on or before September 1, 2014. To schedule your appointment, call Badger Clark at 923-0080. Parents are responsible for bringing the following required items. If these documents are not presented at your appointment, the school will not be able to register or screen your child. Child’s Certified Birth Certificate - Hospital/physician certificates or passports are not acceptable. Child’s Immunization Records - The school understands that your child may not have had all of their shots at the time of registration, but please bring the records you do have. The nurse will inform you which shots will be needed in order for your child to start school in the fall. Proof of Residency - Rental/lease agreement, mortgage/home ownership papers, or a EAFB housing letter are acceptable forms of proof. If you do not fall into any of these categories, please call the Badger Clark office to discuss your situation. Utility bills, paycheck stubs or other items received through the mail are not sufficient documentation. This information is required even if you have other students in the district. Parent/Guardian Photo ID - This must match the name(s) on the proof of residency documentation.
