ETO PO ANG OFFICIAL POLICE REPORT TUNGKOL SA NATAGPUANG NANGANGAMOY NA BANGKAY NG BABAE SA SITIO GUGU STA ROSA CONCEPCION, TARLAC...PARA SA IMPORMASYON NG NAKARARAMI... 1. References: a. Spot Report from this statin dated December 21, 2014; and b. Police Blotter with Entry No. 320 and Page No. 098. 2. These pertain to the Found Dead Female Person on December 21, 2014 at Sitio Gugo, Barangay Sta. Rosa, Concepcion, Tarlac. 3. On December 21, 2014 at around 9:00 AM, a telephone call was received by this station fro, SPO4 Leonardo Laurente y Panagan, Team Leader, VOA Patrol Base, PPSC, TPPO located at Sitio Gugo, Barangay Sta. Rosa, Concepcion, Tarlac that a certain Maximiano Matias y Capulong, 45 years old, married and a resident of Barangay San Agustin, Concepcion has allegedly discovered a cadaver at the Sugar Cane Plantation of a certain Milagros Reyes located at Sitio Gugo, Barangay Sta. Rosa this municipality. Acting on the report, investigators and intelligence operatives of this station were immediately dispatched to conduct investigation and follow-up operation. 4. Initial investigation shows that at about 7:00 AM of December 21, 2014, Mr. Maximiano who was then on board his motorcycle while looking for his alleged missing carabao and when he retrieved the same and passed at said barangay at about 8:30 AM sane date he saw a sling bag and tried to find out what’s inside but when he turned his back he saw a female person lying naked. Immediately he reported the said incident to the VOA Patrol Base. The said found female cadaver was identified based on the documents recovered at the crime scene as Jeralyn Ferer y Valdez, 20 years old, single, presently residing at EM’s Barrio Tejeros Convention, Rosario, Cavite and a native of Mangatarem, Pangasinan. Initial findings by the investigator at the crime scene was that the victim sustained stabbed wounds on her right upper chest and wound on her forehead believed it was hit by a hard object. 5. At about 12:15 PM December 21, 2014, a SOCO Team led by PINSP NOEL LUNA VILLARUEL arrived and processed the crime scene and recovered one (1) black shoulder bag containing the following items: four (4) pieces of Biodatas; one (1) Police clearance issued by Rosario MPS, Cavity dated June 13, 2014; one (1) company ID (Reitoh Cold Storage, Inc.); one (1) Health Certificate ID with No. 13-4754; one (1) Cebuana Lhuillier Debit Card with No. 211040264875; one (1) Birth Certificate; one (1) NBI Clearance dated May 15, 2014; one (1) High School Diploma; one (1) SSS with No. 0234425817 and one (1) Barangay Clearance all in the name of the victim; Biodata of Reymark V Ferrer and of Mary Grace D Valdez; one (1) Vitress; one (1) Babyflo face powder; one (1) Proslo Powder; one (1) Bottle Scent Shop; one (1) Mascara; one (1) Blush on skin goodness avon; one (1) PC earring and one (1) micro SD adaptor. The clothes (brassiere, underwear, pants with belt, blouse) and slippers of the victim were scattered at the crime scene. 6. At about 1:00 PM December 21, 2014, the crime scene was turned over by SOCO team to this station including the items stated-above. The cadaver of the victim was brought to Iligan Funeral Parlor located at Tarlac City for autopsy while the clothes of the victim were brought to SOCO Crime Laboratory for laboratory investigation. 7. Immediately a team from this station was composed to investigate and follow-up the whereabouts if the suspect/s, led by PINSP JOSEPH BAUTISTA MORDEN, Investigation and Operation Officer the investigators and intelligence operatives of this station. 8. The team conducted follow up investigation in nearest barangays were the said incident was discovered to find possible witnesses and identify the suspect/s. Available CCTV footages were requested for review for possible identification of the suspect/s. 9. On December 21, 2014 at about 2:20 PM, a certain Francis Fernandez y Paraan, 26 years old, single, presently working at Tarlac Golden Egg Farm located at Sitio Ding-Ding, Barangay Sto. Rosario, Capas, Tarlac, appeared to this station and verified of there was an incident that this station found a female dead person. When interviewed he averred that he was the boyfriend of the victim. 10. This station presented to Francis the documents and ID’s of the victim and he personally identified that the person in the Identification Card found at the crime scene was his girlfriend who identified as Jeralyn Ferrer y Valdez, 20 years old, single, resident Barangay Caturay Sur, Mangatarem, Pangasinan. 11. As stated by Francis, on December 17, 2014 at about 6:30 PM, the victim surprised him with a visit at the farm where he was working for that date was their 13th Monthsary. Upon seeing the victim, Francis was surprised and confused for that he never told the victim of the place where he was working. 12. Allegedly the victim informed Francis that she tried to locate him by asking every farm near the area if there was a certain Francis working thereat with the help of a tricycle driver who alleged she rented at the Junction of Capas, Tarlac. 13. When the victim arrived at the said farm Francis was working but he managed to brought the victim at the egg rook where they work and victim watched their work routine. Since it was already at the middle of the night Francis offered the victim to sleep for the night at the said farm and promised that he will take him at the bus stop the following day December 18, 2014 but due to some reasons the victim was able to go to home instead only on December 19, 2014 at about 7:00 AM. However, due to the busy day of work of Francis cannot accompany the victim at the bus stop, but the other co-worker of Francis who was then also going home to Pangasinan for some important matters instead joined the victim in the travel who was identified as Jayson Cayago y Nevado, 24 years old, single and a resident of Pampalo, Mangatarem, Pangasinan. Supervisor of said establishment identified as Aries Manalo y Rigor, 27 years old, single and also resident of Brgy. Pampalo, Mangatarem, Pangasinan who also went out of the farm to repair the wheel of the bicycle of his cousin and offered ride to Jayson and Jeralyn at the road side to the main road and be able to ride passenger jitney or a motorized tricycle to the Junction of Capas, Tarlac for them to had a ride on a passenger bus straight to Pangasinan. 14. Jayson and Jeralyn was able to ride a Five Star bus together as claimed by Jayson who sit at the back portion while Jeralyn was seated in front and they arrived at the bus terminal of Mangatarem, Pangasinan and allegedly that was the last time he saw Jeralyn. 15. As stated on their sworn statements, the co-workers of Francis (Jayson Cayago, Aries Manalo and Jayson Peroche) they said that Francis did not went out of the farm between December 19, 2014 to December 21, 2014. 16. At about 5:30 PM of December 21, 2014, father of the victim identified as Jose Ferrer y Bautista, 40 years old, married and a resident of BRGY. Caturay Sur, Mangatarem, Pangasinan, appeared to this station and averred his disinterest to conduct autopsy examination to the cadaver of his daughter Jeralyn Ferrer y Valdez. He requested to transfer immediately the cadaver of his daughter to their house and for them to prepare the funerals of the same. 17. On December 23, 2014 the father of the victim again appeared to this station and requested that he wanted the cadaver of his daughter to be autopsy. Upon request, this station immediately contacted PSINP NOEL LUNA VILLARUEL of the Tarlac Crime Laboratory Office to conduct autopsy examination tit he victim’s cadaver and the autopsy was conducted at Ortega Funeral Parlor, Urbiztondo Pangasinan. The father also gave sworn statement to help this station in its investigation. 18. On December 23, 2014 at around 5:00 PM SPO2 Leopoldo Lugtu, Inves PNCO of this station verified and coordinated for CCTV footages and found-out that the CCTV’s of M’Lhuiller Pawnshop was already installed but not yet connected and in the Infinity Parlor it was under repair. In this place of Junction, Capas where passengers in Capas and most probably where the victim rode the motorized tricycle. 19. On December 23, 2014 at about 9:00 PM, the team proceeded to Tarlac Golden Egg Farm, Sitio Ding-Ding, Barangay Sto. Rosario, Capas, Tarlac to gather information from the co-workers of Francis and for them to give their sworn statements regarding their knowledge of Jeralyn and the incident. 20. One of his co-worker identified as Jayson Peroche y Lucero, 24 years old, single cousin of the victim and also a resident of Barangay Caturay Sur, Pangasinan alleged that one of their co-workers whom he cannot recognized said during their lunch break in the canteen that the driver of the motorized tricycle allegedly took the number of the victim for the purpose that when she came back to the said farm she will just text the driver for her to have a ride. 21. On December 24, 2014 at about 2:40AM the team arrived at the wake of the victim in Barangay Caturay Sur, Mangatarem, Pangasinan for further investigation and to get the sworn statements of the mother of the victim identified as Babylyn Valdez y Dela Cruz, 39 years old and his boyfriend identified as Francis Fernandez y Paraan, 26 years old, single, both residents of the said place.
