HOSPITAL PROJECT UNDER HEALTH CARE & SANITATION BY ROTARY CLUB OF MATALE PROJECTS NAME Renovations and Improvements to the OPD Waiting Room donated by Rotary Club of Matale to Base Hospital n 1975 APPROVAL REQUEST Seeking funds under the DDF amounting to Rs 150000/= and to proceed with the project HISTORY AND BACKGROUND The Base Hospital , Matale was started in Matale to meet the medical needs of the population in Matale district which numbers over 600000 The Rotary Club, considering the humanitarian needs of the grieving public, at the request made by the Hospital Authorities and that of the sanction of the Government, positively responded for the construction a Waiting Room and donated it to the Hospital in 1975. The Waiting room which was originally the OPD waiting room was converted to the OPD for clinical tests and chest ailments due to insufficient space. At present it caters to over 300 patients per day. OUR PRESENT REQUEST The premises due to the ill effects of natural elments, time and other constraints require additional facilities ,such as walls , grills, toilets ,and notice boards. The total cost is estimated at Rs 300000/= approximately. The building is in extent of 40x20 sq ft supported by 11 columns and an asbestos roof. The sides are open . It has rows of cement rendered benches WORKTO FOR EXECUTION Construction of a 5 foot parapet wall ( cement blocks & Complete Rear Wall ) the rear Rs Construction of a 3 foot grill wall at the sides Construction of toilets(commode and two urinals) Construction of drainage Installing pavements. Provision of electricity Provision of Water on tap Provision of Notice Board with Rotary Publicity The club has tentatively fixed a time limit of 45 days and hopes to do the donation before April 2015 The club expects to meet the cost as follows DDF Rs 150000/= Members and well wishers Rs 150000/= The Rotary Club will generate enthusiasm among the Public and Citizens It has a coroporate identity A perpetual advertisement The club has discussed this matter and awaits your kind guidance The present situation of OPD for renovation
