I learned that courage was not the absence of fear, but the triumph over it. The brave man is not he who does not feel afraid, but he who conquers that fear. Nelson Mandela Dear Dr. Hadipour I was thinking of Iranian Blood Transfusion Organization on one hand and the health ministry on the other. I am not an IBTO fanatic and I am not in the position of defending one or the other or not either trying to find a way to redefine their relation; never! But I want to talk about is based on realities, based on truths. I have been always witness to the scenes in which health ministers particularly this recent one paying visits to IBTO with the consequent sort of complaining tone and unsatisfied language. What for? For a large flow of people in long lines waiting to donate their blood and in fact their human values. Right! Great! Altruistic people are doing their job fine and the minister is doing it fine as well. The incompetent, the inefficient party has been always IBTO. It is IBTO to blame, IBTO to be the wanted, IBTO humiliated, IBTO left ignored or left alone to deal with its feeling of guilt and unresolved problem. Why? The answer is the facet did not look good, so many awaiting, people complaining, and the stuff like this and so the minister of health did not like this. But one thing is missing and that is unfortunate to say; the silence of IBTO. I do respect minister for his diligence but one important thing is missing and that is the timing: the holy days are not the right times for the evaluation of blood services either in Iran or out even in the developed countries. So minister is wrong in timing. It clearly shows that minister of health has a poor perspective of what a blood service is and how it should operate, what the norms are, what the scope of its duties is and most importantly what it whereever in the world it is should be expected to do and what to its greatest extent can practically do. Next the buck should be passed to IBTO managers that have either kept silence to inform the health minister of how wrong he is or been unaware of this clear fact; I hope the latter misery would not have been true. I hate this silence or unawareness. I am not saying that minister should not watch IBTO and neither am I exonerating IBTO managers in not having a plan to encounter the big donation events. Nope! But we should know that we should do the right thing in the right time. We should help ministry of health to know big issues about the blood system not just about blood bags and the staff but in changing the wrong expectations and making preparations. That is the main goal of IBTO that has not been addressed correctly at its age of 40 when one should be wise enough to move more effectively forward. Make minister aware of the realities, tell him the whole story; tell him IBTO has been wrong in being creative enough to think ahead and tell he is wrong in being not adequately aware of blood system proper functions. Do not be afraid of anything!
