Midwest Central Announces Registration Dates and a New Streamlined Process for 2014-15 School Year Office of the Superintendent -- The Midwest Central public school system announced changes to its registration process for the 2014-15 school year. To make the process easier for parents and students, the location of registration will now be held in one place for all three buildings in the school district. The school district is offering two dates for registration, with morning hours one day and late afternoon on the second day: Dates: Wednesday, August 6th from 2:00-8:00 p.m. and Thursday, August 7th from 9:00 a.m.-2:00 p.m. Location: Midwest Central Primary School, 450 Southmoor Street, Manito, IL 61546 To expedite enrollment, parents are asked to bring documents listed below to registration and take note of the following information: All students (district-wide): • New students are required to have proof of residency, a parent photo I.D. (at the Primary School only), a certified birth certificate from the county in which they were born, and immunization records. High School students: • Incoming freshman should have updated shots, including proof of 2 Varicella booster shots, along with a physical exam. • Basic book fees will be $80.00 (other fees will apply and vary by courses enrolled in) Middle School students: • New students will need a certified birth certificate from the county in which they were born and physical. • 6th grade students are required to have physicals, a dental exam, and the appropriate vaccinations (prior to entering school as required by State Law) • A sports physical is required for all Middle School students who plan to participate in sports. • Fees: Basic Book Fees - $85.00, Band - $7.00, PE Shirts - $5.00 each (students are required to provide their own black shorts), and 7th grade students and new 8th grade students to the district will be required to also pay a $7.00 fee for Constitution Books. Primary School students: • The Primary School Book Fees will be $65.00 for K-5th and $20.00 for Early Childhood. • First time students and kindergarten students are required to have a physical, eye exam, and proper immunizations. We accept cash, check or credit card. The Mason County Health Department will be at registration on Wednesday, August 7h from 9:00 a.m. - 12:00 p.m. to host an immunization clinic. Students must be accompanied by a parent/ guardian to receive immunizations. For more information, parents are requested to contact the school directly. Offices are closed the month of July and will reopen Friday, August 1st.
