These are the nmemonics I learned a while back, there might be some difference between how doctors and EMT run things, but the concept should be universal. If youre more comfortable with your philosophical homie, Socrates, by all means. OPQRST: Onset--When did you first notice symptoms/when did injury occur? Provocation--Does anything you do make it feel better? Make it feel worse? (eg. tenderness when pressure applied, rebound tenderness when pressure is removed) Quality--What type of pain? (shooting, cramping, etc) Radiation--Does the pain radiate to any other part of the body? Severity--Scale of 0-10 (10 being the worst)? Time--How long have you been feeling this pain? Is it intermittent (come and go)? SAMPLE: Signs/Symptoms Allergies Medications--perscribed/over-the-counter Past medical history--previous illnesses, doctor visits, tests Last oral intake (applies more to patients who seem sluggish, get an idea for hydration and glucose level) Events leading up to illness/how you got your injury
