The technique of preparing and filling 1mm short of the radiographic apex is something that should be consigned to the history books! The last 1mm of a root canal CAN of course contain bacteria and also the apical delta (where the canal branches!) just being 1mm short could actually be 1mm with 4 branches apically meaning essentially 4mm short!! In modern root canal treatment we should aim to prepare the entire length of the root canal with small hand files (size 8 or 10) and place the apical stop (seat) as close to the apex as is practical (usually 0.5mm from the apical foramen). Radiographs are basically a poor indicator of the true canal length and so a combination of radiographs and an electronic apex locator should be used.
