There are packets being mailed to IHSS Recipients containing important information (form TEMP 3002). There is also a form (TEMP 3000) that needs to be submitted by December 15, 2014 to your local IHSS Office (It should be shown at the top of the cover page received with your address). Also sent was an example of the new timesheet form SOC - 2261.
