Conditions such as infection or shock may lead to a transient ischemia, followed by the reestablishment of blood flow (reperfusion). The ischemic damage may generate free radicals, and the reperfusion may provide O2 to combine with free radicals to form ROS. Ischemia/reperfusion injury activates neutrophils to release ROS and hydrolytic enzymes, and formation of NO is induced. This in turn causes an increase in injurious peroxynitrite (ONOO) formation. Inflammatory mediators such as tumor necrosis factor-  (TNF-), interleukin-1, and platelet-activating factor may also be released.
