Introduction Sarcoidosis is a multisystemic disease and can be confused with benign or malignant tumors. It has been reported as a cause of false-positive cases in staging of cancer patients with FDG-PET. Case presentation A 58-year-old woman during the follow-up for a stage IB endometrial cancer presented at our Institution with a suspicion of progression of disease at PET/CT without other signs. Considering the low probability of metastatic lymph node involvement from endometrial cancer, we decided to do a biopsy of lymph node that was diagnostic for sarcoidosis. Conclusions The association of sarcoidosis and malignancy represents a critical issue of staging and diagnosis as a result of concurrence of these two conditions. The clinical implication of a correct diagnosis is clear: an unnecessary anticancer therapy could be avoided instead of appropriate steroidal therapy for sarcoidosis. intermedcentral.hk/index.php?journal=jss&page=article&op=view&path%5B%5D=89
