KEY POINTS ? Subacute thyroiditis has a postviral cause; anterior neck pain and an elevated erythrocyte sedimentation rate (ESR) are common; the patient generally recovers to a euthyroid state. ? Painless thyroiditis has no neck discomfort; it is called postpartum thyroiditisif occurring within 1 year of delivery or silent thyroiditis if not associated with childbirth. It is likely autoimmune and can lead to long-term hypothyroidism. ? In thyroiditis, there may be four phases: hyperthyroid, euthyroid, hypothyroid, and recovery to euthyroid state. ? In subacute and painless thyroiditis, uptake studies show decreased uptake of radioiodine. ? Other rarer types of thyroiditis include radiation thyroiditis (after 131 I administration) and acute and subacute infectious thyroiditis. ? Infiltrative diseases that involve the thyroid are rare but include Riedel’s thyroiditis, amyloidosis, and sarcoidosis.
