The complex issues involved in designing appropriate incentives in health care is partially laid bare in this article. This is why I believe decision making must remain in the hands of physician and patient, and standards of care must be guide by physician groups, not insurance companies, who never meet the patient, and care for nothing but their bottom line as a rule, as per their corporate legal responsibilities.
