My former assistant Aingyea Kellom can attest to the fact that when someone was afraid to tell me about a mistake or error that they made in the workplace, my first question was typically Did someone die? If the answer was No - and 100% of the time it was - my next focus was the corrective action necessary, not the mistake a staff person made. We ALL make mistakes and unless someone died or there was a substantial loss of money (and I mean it has to be a substantial loss), I usually moved on though I believe I made it clear that we needed to avoid a next time. And typically that particular sort of mistake was never made again.
