(health) and ask why they have not paid any on it yet, after some research they tell me they will now process the claim, then 5 mins. later they call me and say that the forms they sent me months ago is missing the last page and they need a copy of the crash report. I go online and get the missing page , now to the ncdmv site where I get a copy request form and have to mail it to Raleigh with a check for $5.00 to get the crash report !
